1. Legal Stuff

For legal reasons, we ask that people who are working the 'Fest be at least 18 years of age. If you will still be under 18 by September 22nd, 2007, you must fill out the hardcopy version of this application and have a parent or guardian sign it for consent purposes, and mail it in by September 1st. WE WILL NEED TO SEE YOUR PHOTO ID AT STAFF CHECK-IN FOR AGE AND LEGAL NAME VERIFICATION.

By submitting this application I agree to hold Cannabis Revival and Joplin NORML harmless if something happens to me, such as injury or damage to my person or property, while volunteering for Cannabis Revival or Joplin NORML. This includes any activity whether the incident occurs before, during or after the event or if the incident occurs within the boundaries of the event or outside of the event's perimeter. In addition, I assume liability for my own actions if I cause injury to any person(s) or damage any property.

Do you have a vehicle, valid driver's license, and insurance to provide transportation or run errands? (Sorry, there is no reimbursement fund for gas expenses.)
